Health Risk Appraisals
The Personal Wellness Profile TM (PWP) is recognized as a comprehensive health risk appraisal available for individuals and corporations. The PWP is designed to appraise the health risks of the participant and provide an aggregate corporate report identifying the health risks and needs of the company. Each participant's current health status is evaluated through a self-reported questionnaire and selected testing. The PWP can be administered at several levels, from basic appraisals to executive physicals. Each participant's results are sent confidentially to the participant. In addition, aggregate corporate reports that summarize the health status of your company are provided.
The following tests can be added to the PWP for reporting purposes:
- Blood pressure
- Pulse
- Height/weight
- Lab work
- Body composition
The PWP is administered online and is available in Spanish.
Health Coaching
Health coaching is an excellent follow up for employees to the health risk assessment. The health coach can review the employee's individual PWP and continue with one-on one discussions with employees regarding improving one's health status, goal setting and lifestyle changes throughout the year. The health coach is certified by the American Council on Exercise (ACE) and is available to participate with wellness committees to help develop company wellness programs.
Health Fairs
Health fairs provide valuable health information and screening services to employees. Armed with the information provided at health fairs, your employees can learn how to get and stay healthy. Interactive and informative education options can be tailored to meet your organization's needs. Sample services that can be provided include ankle bone density, healthy eating and skin cancer checks.
We also offer pre-registration for our corporate mammogram program in a health fair setting. With this program, we offer blocks of time just for your company to receive 3D mammograms at Lutheran Kosciusko Hospital Women's Imaging. Because the registration is completed at your health fair, your employees will only need to sign their consents when they arrive. These appointments are spaced just 15 minutes apart so that your employees are seen quickly and back to what is important to them. Along with the mammogram, a complimentary body scan and refreshments are also offered. Billing goes through the employee's insurance.
Lunch and Learns
The Lunch-N-Learn program is to provide health education for corporations in a group setting. This type of programming allows sessions to be presented during employee work hours without having to schedule or pay for additional time on the job or decreased productive time. The sessions last approximately one hour, but can be modified to match time requirements that work for your organization. The employer should be aware that employees often have questions and stay after the program is completed. The topics are customizable if you don't feel there is a topic on our list that would meet your needs. Sample topics available include guidelines for a healthy diet, balanced fitness and reading food labels.
